Dog Intake Request Form

Please answer all questions as honestly and thoroughly as possible.  Submission of this form does not guarantee that we can accept your dog into our program but if we can’t, we will try our best to direct you to other sources.  To expedite this process, please email pictures of the dog you are needing help with to email@tarasdream.org.  If we are able to accept your dog into our program please note there will be a $30 surrender fee to help offset our costs.
